It is mandatory for the concerned service provider to have Food Supply Registration / FSSA Act-2006 license certificate. 4. Name, father/husbands name, Aadhaar number and address of the mess manager, chief cook, assistant cook and other staff appointed by the service provider to prepare food and refreshments for the players participating in the competition organized by the District Sports Office, Mau. The list of insects will have to be compulsorily made available to the Insect Officer Mau and it will be mandatory to get their health test done. 5. The eligible service provider will have to prepare and distribute food and refreshments in the mess every day as per the fixed time/menu and the menu details will have to be flex printed and displayed at the gate of the dining hall and at other appropriate places..
